I have a Dvico Fusion MCE USB remote. I'm using R5C7 and installed and patched LIRC from the CVS repository as listed here http://www.mythtv.org/pipermail/mythtv-users/2005-August/100371.html.
After doing this, IRW recognizes every button on the remote. However, it doesn't work in MythTV. Only a few buttons are recognized (mainly number pad). But still, MythTV doesn't recognize any more buttons than it did before. Even in the Keys Setup, only the number pad and the Play/Pause, FF, and RWD buttons are recognized.

What else do I need to setup?
