I need to call nvtv on each boot to adjust the tvout. Where should I put the line? I think it would be fine to have it in the same script that starts mythtv frontend so that I can run nvtv just before the frontend.

great ... now for anyone else out there with similar issue
add
exec nvtv -r 800,600 -S NTSC -s Large -t -o 25,0
(or whatever your nvtv command is or any program you want to start with X)
at the end of
/home/mythtv/.fvwm/.fvwm2rc

i wasnt paying attention but an easy way to find out is add an eco line in it like
echo "HEY gr8nash bootmisc.sh starts here"
in the top of it :) then reboot and look for it.

well look at the number on it in /etc/rc3.d dir. they go in order ones that start with S are run ones that start with K are not. I believe GDM runs as S99 which is the last. the problem you have sometimes is there will be 5 things at S99, and I'm not sure how it decides which one to run first, but I think at that point they run pretty much parallel and one doesn't have to wait for the other to finish.

I'm sure that's all clear as mud, but think about it and look in the /etc/rc.? directories and I'm sure you will figure it out.

Wow, I haven't heard anyone else use that expression in ages... It's probably not surprising that it was an old TN Volunteer that infected me with that meme in the first place...
