Total cost: $475.83 shipped

Using the proper Ultra DMA cable for the DVD drive, everything worked right away. I think that I was even able to use the streamzap during part of the setup. I'm happy so far except for the volume. I belive the main source of sound is coming from the 2 60 mm case fans, so I will hopefully replace them soon.

I had actually purchased a DVR-112D from newegg, but I swapped it with the DVR-111D I had in my desktop because I had problems reading from it. Turns out I was using the wrong cable. The one marked "CD-ROM cable" is the wrong cable to use. You want to use the "HDD Cable". Hopefully this is useful :)

BTW: I plan on writing my step-by-step "tutorial" with hardware photos soon, so if you have further questions, look for it :)

Sorry for the lack of response. The one listed in my original post is what I used for SD only. When I upgraded to HD I replaced it with this:
Patriot 1GB 240-Pin DDR2 SDRAM DDR2 800 (PC2 6400) Desktop Memory Model PSD21G8002 - Retail - 11.99 (Newegg)
At the same time I upgraded the processor to:
