My setup with the PVR-350 works fine now, however I can't use the remote to control volume or mute. The change gets displayed on the TV, but the volume stays unchanged. All other remote buttons work.
Using test_ioctrl at the commandline does work. So it seems that there is something missing between the lirc commands and the SW that does the action.

I am using R4V4, a PVR350, the TV hangs on the TV-out of the PVR350.

well test_ioctlr actually controls the volume level on the pvr card, which really should never be touched once you've got it set. In the mythtv setup you can chose what you want the volume control for myth to adjust. by default it's master volume, which generally works, but for some reason some soundcards this doesn't work. I would suggest playing with alsamixer till you find what actually controls the volume then change the setting in mythtv to that.

I tried both available settings for the mixer, but none did work.
Is the sound card used at all if the sound is merged into the video directly inside the PVR-350? I assume that the soundcard is not needed at all (I think I even tried it).

well the sound card is used on playback. unless you are playing everything including sound back through the pvr350? I think I'm just going to quit replying to the pvr350 posts, cause every one of you is looking for a different setup.

I am sorry if I caused confusion.

The only connection I'm using is the one from the PVR350 to the TV (composite video + stereo audio = 3 cables, but SCART works as well). This means that audio comes from the PVR350.

What should be the way to control volume & mute in this setup?

hmm in that case probably the only way to control the volume would be the tv volume or test_ioctl which I don't think can be integrated with mythtv, although I am not sure about that, because I do not have a pvr350 and have never looked for that.

I think some people use an adapter cable to feed the audio out of the pvr-350 to the sound card input and then from the sound card output to the tv. This will enable MythTV to control the volume.

In my set up, I use a universal remote to control the volume/mute/power of the tv. However, I only use MythTV so having the output of the pvr-350 directly coupled to the TV is not a problem for me. If you want to hear sound from other modules also, I think the pvr-350 output->soundcard input->soundcard output->tv/stereo is the way to go.

only problem with that is I haven't heard of anyone getting that setup working properly.
